Acne - Definition Most people develop acne - the most common skin condition -- to some degree, but it primarily affects teenagers undergoing hormonal changes. Acne may be mild (few, occasional pimples), moderate (inflammatory papules), or severe (nodules and cysts). Treatment depends on the severity of the condition. Acne is primarily a hormonal condition driven by male hormones, which typically become active during the teenage years. Sensitivity to such hormones, combined with surface (skin) bacteria and lipids (fatty acids) within sebaceous (oil) glands, cause acne. Common sites for acne are the face, chest, shoulders, and back -- the sites of oil glands. Acne lesions include comedones (whiteheads, blackheads), papules and pustules (small bumps), nodules and cysts. Acne - Myths Many myths have been linked with acne, particulary role of stress, diet, poor hygiene, and premature use of cosmetics. In a survey of 178 acne patients and relatives, 74% believed anxiety was an exacerbating factor, yet few objective data support this nition. The standard of acne care in the 1950s included a diet with little or no chocolate, sweets, fats and carbonated beverages. Since no clear evidence has shown diet causes or exacerbates acne, this recommendation is no longer valid. Although many individuals still associate acne with poor hygiene, removal of surface sebum or bacteria does not improve acne. In fact, frequent or harsh washing may exacerbate the condition. Cosmetics appear to be relatively harmless, at least in the short term. However, patients should be advised to use noncomedogenic, non-acnegenic, nonirritating, hypoallergenci products - which are the only products Boston Dermatology physicians recommend. Topical Therapies Topical acne solutions are availabile in a wide array of formulations and concentrations. Proper selection of appropriate solutions to minimize adverse effects will increase the success rate. In general, creams are greasier than other formulations and should be used primarily in patients with dry or sensitive skin. Gel formualtions are typically reserved for patients with oily skin, while lotions are recommended in a variety of settings.
